Nah mate it doesn't actually have anything to do with Centrelink, it's actually the Department of Human Services. They share a website with Centrelink but this is totally seperate.
Okay here are the details for you from the bottom of page 2 of the forms you have filled out. The mailing address is there and I have included a link so you can look it up if ya need.

| Yep, I've done all that, but when I went to lodge them at my local Centrelink they said I don't qualify because I don't get benefits???? Maybe I'll try calling the Helpline instead! Kelly | |

Welcome to the site.
You need to print out forms for your GP to fill in. Here's a link to the Gov website.

It MUST state that you have a life threatening condition, and co-morbidities.
Your Surgeon can write a letter or fill out your forms, again it MUST state "life threatening".
I'm pretty sure your surgeon will know exactly what to write. They will also give you a full quote of exactly what you will pay and to whom and that is what you claim on the forms.
Oh, make sure when you do the expenses form that you have absolutely NO savings in the bank and that your outgoings are more than your incomings.

The forms are on the centrelink page but it's a completely different department.
There is a lot of information regarding super release in this forum .
I don't know if anyone that has been knocked back unless they didn't have enough to cover the op.

| I tried to apply for early release of my super, when I searched the internet it said to apply via Centrelink. When I went there they said I don't qualify because I am not on Centrelink benefits....? Am I missing something - is there another way? Kelly. | |
