Hi Kellie_45,
No worries, I hope there are no issues with mine too, i'm booked in for surgery on the 18th of June but I have to pay by the 19th of may so i'm hoping it's all done by then otherwise i'll have to re-book my surgery date which I really don't want to do, I've waited long enough already waiting for my PHI to kick in lol
Hope it all works out for you so you can enjoy your cruise :)

Hi Iheartdaniel

Thank you for the information, I phoned DHS this morning, they have my application but it wont be reviewed until 13th May because they have a back log, Im hoping that there are no issues and tthat I receive my approval letter very quickly because then I have to send the letter and withdrawal form to my super fund which will take another 10 days before the funds are put into my account. Im waiting to hear back from my surgeon to see if I can be booked in for mid June - I rreally hope it all works out for me, we have a cruise booked for September and I want to be recovered before we go away

I was just reading through this thread and wanted to ask you did you have any trouble getting your early release approved for the full amount? Also if it has been approved how long did it take?

I applied a week ago for the full amount as I am not in a Private Health Fund and like you I am a little anxious

If anyone else can help I would be very appreciative

Hi BringingBellaBack,
I sent my forms off about 2 weeks ago and I still havn't heard back, apparently because of the easter break the turn around is now 12-15 days. I did call the the day after I emailed everything (thursday, day before good friday) to see if they received it and the lady I spoke to said it will take till the following the wednesday to make up a file for my early release of super and that it should take about 10-12 days after that, so I will give them a call next wednesday which will be my ten days, Im like you very anxious :)

Hi Lioness!

No I didn't have to provide any of that Applied for early release of super this week. 65317 lol (i'm sure you've seen...) in the application you are asked to provide a summary of your income and budget, if you're married or in a defacto relationship your partners income (I just said I was single), assets, liabilities etc.. and it's best to make sure you look like you have NO other way of paying for the surgery because they can be picky and suggest you sell off assets etc.

All I was asked to send was...
- the application
- POI 100 points verified (I had mine done at a pharmacy)
- letter or completed form about your "life threatening condition" - 1 x from GP and 1 x from specialist
- info/quote on the price.

If you like I can forward you the email I was sent with all the required docs attached? :)

Hi MissHollywood23, Im in the process of completing my super forms and was just wondering if you
had to send any health cover rebate details? im stuck on the question where it asks
"do you have private health cover which will assist with any or all of these costs?"
are they meaning just the cost im claiming super for or the entire total..
im only claiming for the gap and anaesthetist as my hospital fees will be
covered by PHI and i'll just pay the hospital excess..

thanks in advance :)

How long did it take everyone to get their super application approved. It's been 2 days and I'm impatient already! Haha

I'm applying for the entire cost, and nervous they might say no..
I earn good money, but have a lot of debt and very little disposable income :(

Hi Sohobear,

My GP had no idea either about the release of super.  My surgeon wrote a detailed letter instead of filling out the form, which DHS is fine with.  I showed my GP the surgeons letter because they deal with this often, so GP basically copied this letter in her own words.  I would suggest getting your form (or Letter) from the surgeon first and then taking it to the GP so they know what needs to be written.  You should find your surgeon has done plenty of super forms or letters before and will know exactly what to say.

Hey everybody! I am applying for early release of my super because I would like to get the sleeve done. I have an appointment with the surgeon on 18/12/2013 and am booked in for surgery 6/02/2014.

My BMI is 35, I have PCOS and Insulin Resistance, have been overweight/obese most of my life. I thought everything was smooth sailing until I asked my GP to fill in the early super form. She has never seen or done one before and declined. I am seeing another GP this week who I haven't seen in nearly a year.
Just some advice on how I can educate this GP if he hasn't seen one either? I will get a letter from my health fund saying my plan will not cover the surgery. What else should I provide to help my case for funds to be released?

Thanks :)

So happy, I got my approval letter today!!!

Will send my application to Qsuper tomorrow. Has anyone dealt with Qsuper with this and if so did they take long to release the funds?

Hi typo :)

On the form that DHS required you to fill out there is a section for your budget (weekly, fortnightly and monthly expenses). I believe they want to ensure that there is no room in your budget with the money you have left over at the end of each fortnight/month for you to pay by the surgery via other means. As I work in education this wasn't an issue..lol :) You don't need to provide pay slips or bank statements, though they will ask how much money you have in the bank. I hope this helps!

Hi all- congratulations to those that have had their applications approved! I have only just started looking Into the process and I am wondering what other documents you need to supply?
I know you need the 2 doctors letters and obviously an application form (from DHS I'm assuming) what else is requested in terms of your financial position? Do you just need a budget or are bank statements and pay slips also required??
Thank you in advance!

Hi Guys,
Thanks for responding to my problem. I had the surgery on Wed, and I went home on Friday, full of more wind than usual, but home YAY.
I had an argument with my surgeon on the Friday, which is kinda worrying, but he still refused to write those three little words. I got my refusal letter on Wed ( day of surgery ). My surgeon reduced his fees, so not all is gloom and I just have to think positive now. I am thinking of appealing the decision, has anybody any experience in an appeal.

Thanks

Awesome news misshollywood, I just hope the postie doesn't send my letter to the wrong street like he does quite often, lately I've been getting other people's mail from streets nearby. Hopefully there are honest people if it does happen

I have been approved as well! :) So just to wait for the letter to come in the mail and she said it's a good idea to contact the superfund/s now as well and let them know the letter is on its way and to see what their process is as they might require ID and bank details etc.

I'm so excited! its all happening!! I need to study hard for an human bio exam on monday but i'm just so excited... woooosaaaaaaah >< lol

Just rang DHS and found out my application was approved on tuesday....YAY!!!!!
now i just have to wait for the letter to arrive, looks like the postie has been already and its not there yet.  Does anyone know how long it took from approval to receiving the letter?  the lady told me it was likely sent out wed or thurs, but i'm wondering if it sits in a pile for a while first before truly being posted.

MissHollywood....any news on your application yet? you had yours in way before me

my gosh! my surgeons office write an amazing letter about it being a life threatening condition and even have references in there to different studies.. it was almost like a little literature review/essay!!

I hope this means i'm not going to have any issue!

I'm sorry to hear your being put through such a stupid situation PhiL! And I see today was supposed to be your surgery date :( I would definitely seek another surgeon! this one sounds stupid.

Good luck!
