a fatty liver is more the norm than a oh my gawd shock thing when they test prior to weight loss surgery. The optifast diet prior to surgery is to help with that fatty liver. Yes I have heard of people who got on the table opened up then closed right back up because their liver had not shrunk. But NOT a single one who had done optifast for the amount of time the surgeon instructed them to without cheating. Talk to your surgeon how important doing the optifast is depends on a few things. first your surgeon, some even do surgery on the biggest liver patients without it. Your shape if you carry your weight on your stomach it can mean longer optifast. previous surgeries, this can mean a mess to get around inside, again some surgeons may want to play it safe. your weight, 100kg and under a lot dont even do it, over 150kg nearly all do even with all other things being good.
Listen to your surgeon and do what he wants and it should be fine. if it will make you feel safer do an extra week of opti and get a bonus head start on weight loss :)
