Congratulations on your date and super release.
Don't stress out too much, the surgeon who has given you the date should pretty much do everything for you.
They should send out a pack with all the info you need and should send you hospital admission forms or they will inform you how to do it online. You then send those to the hospital direct.
Good luck with it all, but if you have any questions or are unsure, you should definitely call your surgeon.

YAY!! so very excited! my super release was approved last week and my surgery is booked for June 10th! Let the journey begin :-)  I am a little confused as to what I dof rom here but?? Does the surgeon book me in to hospital or is there something I should be doing?? I am going to call the surgeons office today and see if they can help me...I have never been in hospital before or had any type of operation so its all new to me!
