Thank you both - interesting the food intolerance led me down a few dr google paths and given some of her other health issues and the heart attack 24 hours post op Vagus nerve damage may be a contributing factor... watch this space!

Has she been back to see the dietitian?  it sounds like food intolerance to me.
It could be to FODMAPS

Your poor mum. It sounds awful!  I wouldn't worry too much about the increase in meal size as causing any damage.  My dietitian had me on 1/2 cup from the beginning of solid food and I have not had any issues at all - took me a few weeks to get to that but I did.

I don't mean to sound simplistic but I have heard that some people experience food tolerance issues after the sleeve. I'm pretty sure that things like lactose or gluten intolerance can cause some severe pain and discomfort for some people.  Maybe that is an avenue you could check on if you haven't already.

I hope the doctors can sort something out for her soon.

Has anyone heard of or experienced this at all? 

My poor Mum had her band removed, gall bladder removed and sleeving done in early Nov 2014. She had a heart attack a day post op and after getting over the shock of that did really well for the first 4 weeks. On advice from the dietitian she increased the size of her meals to 1/2 cup (I was shocked given my sleeve 12 months earlier and I didnt budge from 1/3 cup for nearly 6 months) now she has significant pain, acid and her stomach is constantly filled with gas - so much so that the sonographer doing her ultrasound today couldn't see anything until she had burped a number of times. 

She has had two gastroscopies now and many blood tests etc and is feeling severely unwell, fatigued, constantly nauseous. Her words to me today was that this was the worst decision she made and she is existing not living as she needs a rest after going 10 steps to the toilet. Therefore she is scared to eat and drink because of the pain and acid, the surgeon has upped her nexium and she has stopped the aspirin for her heart in case it was upsetting her. 

I have read some of the posts on here (Suzy77 and Maryle) about their journeys, but has anyone else experienced such significant complications? 

My husband, myself, a close friend, her mum and my husband's boss all have had the same surgeon as my mum and he is brilliant and none of us have had any of these complications. For my husband and I this is the best thing we ever did, our lives have changed for the better.

Guys - any help or suggestions - did anyone experience anything like this - what worked, what didn't, we are absolutely clutching at straws as to what to do for her now and she is giving up hope.
