Approval for Super.. How long do i have to claim?

I would ring the Super Authority (whatever they're called) and tell a white lie. Say you had some family circumstances that prevented you being able to take time for the surgery and ask them how long the approval lasts for.

Hey guys

so I was approved to release my super about 7 months ago however I has a bit of a freak out and decided not to go ahead. Now i am regretting that choice and would like to know if i can still go ahead with the claim. I have the letter but I'm not sure if i now have to send off a new claim or if i can use my original letter?
