| Hi Miss Krys,
I have recently had my super released, the GP has to fill in the DHS form as well as writing a letter. The best thing to do is get the surgeon to do it first and give a copy of this letter and the form to the GP and get him to do the exact same thing, thats what i did and I had no trouble. My GP just had an issue writing that I was going to die if i didnt have the operation so he wrote may die, but this was accepted no problems.

Have a read through the DHS forms and see what the current requirements are. I believe the GP can just write a letter. But the forms as they were more comprehensive.

| Hiya Miss Krys,
Early Release of Super is through the Department of Human Services.
They have forms you print out for your GP to fill in. As long as it says "life threatening" illness or co-morbidities.
