From band to sleeve... will my insurance cover it?

Hello. I am with GMHBA.

In March 2009 I had lapband surgery which I paid $5000 out of pocket for and the insurance covered the rest.

I am considering having the band removed and changing to the sleeve.

Does anyone know if...

a) insurance covers removal of band?
b) GMHBA will cover the sleeve replacement?
c) what the out of pocket expenses are likely to be?

i have gold cover with a $500 hospital excess.
