| Hi
I am hoping someone might be able to assist.
I cannot find a GP on the NSW Central Coast to sign off on the early release of super form! They don't believe that it is life threatening :(
I have a BMI of 41 and comorbities PCOS and insulin resistance.
Does any one know of any GPs on the central coast that are happy to sign the form?
Really appreciate any help!!!
