Care Plans??

it is worth chasing a Care Plan but as Denzel said - you get up to 5 visits (divided between allied health doctors) PER YEAR. You can get care plan done every 12 months.

One thing you need to do before seeing dietitian or exercise physiologist is to find out whether they are accepting Care Plans first, then tell your GP to do plan with their name on it.

Some Allied health accept CP and you are fully bulk billed, some accept and you still have little gap to pay and some will not accept them at all.

Hi, the "care plan" is done in conjunction with your GP and the medical centres Primary Health Care Nurse (different from the Practice Nurses who do immunizations, dressings etc) to help the patient manage chronic medical conditions such as MS, arthritis, auto-immune diseases, diabetes, hypertension etc.
The name of the plan is a "Chronic Disease Management Team Care Arrangement".

Even if you qualified under one of these due to your existing and on-going health problems, the number of visits to allied health professionals all up has been cut down to a total of 5.

I went through Western Surgical - Dr Sue Taylor. Standard package, including the surgeon etc.

Highly recommend her and Dr Harmdorf. Both excellent and great to deal with.

My care plan came with my surgeon :D 12 months of dietician, gp, psych and exercise physiologist. Guess it depends on which surgeon you are going to :D

Otherwise you can get referrals to suitable dieticians etc. 

Having a care plan is great, I have found that i have needed to email my team with questions and concerns
