That form you have linked is the correct one, and as Eclectic has said, you tick compassionate/specified grounds. Fill the details in, attach certified proof of ID and a copy of your DHS letter and you should be sweet :)

Ok I have been approved for the release of my funds by the DHS (letter is in the mail)
What I am wondering (as I have searched high and low) is when applying to the super company what forms did you need to use, I rang my fund and they said to use the "Financial Hardship" form but that does not make sense as part of it states
For investors aged under 55 years and 39 weeks: • you have been in receipt of an eligible Commonwealth income support payment for a period of no less than 26 continuous weeks, and • you are able to demonstrate severe financial hardship
I am not receiving any Commonwealth income support at all.
Has anyone dealt with Essential Super at all or just any information at all would be great on what form you used with your super fund.
