| The quote from the hospital would be needed if you are claiming hospital costs out of your super. If you have a health fund that covers your hospital costs, you dont need to provide this :) | |

| What quote did you use from the hospital? I haven't got one! Only the anaesthetist and surgeon! Also do you know what to attach where it says 'evidence of private health rebate' ? Thanks in advance! | |

| That sounds right to me but it's a few months now since I did it! If I remember correctly, there's a checklist at the end of the form so triple check against that. Good Luck! | |

| Ready to send my forms off for super, and I'm wanting to triple check that I have everything I need!
So I have my letter from the surgeon, the form from the GP with 'life threatening' ticked, 100 points of ID that have been cited and JP'd, the 10-page form that includes budget (which I've made us in the red at the end of each month) marriage cert for proof of dependency (claiming hubby's super), signed quotes from surgeon, anaesthetist and hospital.
Am I missing anything major? Does that sound about right?
