I just got my surgeon to fill out my form today and he was more than happy to do so and now just need one from my gp, what were some other details/paperwork you sent that could help me, I'm booked in for surgery on 18th June and wanting to send off my paperwork asap

Hi all,

I have decided to apply for early release of super through my hubby's
super to help cover costs for my sleeve. I need some help on
the form where it says I need evidence of dependency, has anyone
accessed their partner's super and what evidence did you
provide for dependancy?
