help needed with Q55 DHS form

Question 55 tick other and right in morbidly obese and life threatening make sure the doctors and the specialist have written almost word for word on there letters mine was approved in 10 days and I got the money with a week after that so good luck

Super is now applied for through the DHS (department of human service) forms can be obtained from medicare or centrelink sites.

"Administration of the early release of superannuation benefits on compassionate grounds has moved
Administration of claims for early release of superannuation benefits on compassionate grounds transferred to the Department of Human Services (DHS) on 1 November 2011." Quoted from APRA site.
There are 2 forms one for the person whos super it is (this is 11 pages long with 75 Q's) the other is for the GP and surgeon to fill out (must return 2 copies one for each).

Q 55: Please identify what the expense(s) you are appyling for relate to.

A: Ongoing treatment (eg physiotherapy, psychiatric treatment etc)

B: Medication

C: Dental Treatment

D: Other ............... Give details.



I am thinking it is D: Other?

The super forms from ERSB dont go up to Q55 im pretty sure of this.

Hi I had my super approved.My lady Dr wouldnt say it was life threatening though so i went to my Dr b4 her and he wrote what I told him.

I sent everything in and they rang and said I will need my marriage cert they didnt say that in the first place.So sent that in and approved the next day.

What is q55??

Hi all

I am unsure how to answer Q55 on DHS form. Is it ongoing treatment or other?

Also If someone doesn't mind can you please tell me about a spreadsheet (whats needed/included).

I have all the paper work for application to DHS, a copy of the template for GP letter, Birth cert/passport/marriage cert.

Is there anything I am missing? I just want to get this right so I don't slow down the process.

and finally roughly anyone know what is the ratio of approved verses denied in regards to super?
